Options for Community Living
The Options Program provides long-term care information and assistance, homemaker/personal care, and home delivered meals to adults with disabilities who do not have services available to them from other programs.
Long-term care information and assistance services are available without restriction to all adults with disabilities and their caregivers.
To be eligible for homemaker, personal care, and home delivered meals, an adult must meet activities of daily living and instrumental activities of daily living limitation requirements. In addition, for home delivered meals, adults with disabilities age 60 and older must be homebound.
There is no income eligibility requirement for these services. However, there is a sliding fee scale based on income.
